Le verbe échouer est intransitif. C’est un calque de l’anglais to fail an exam que de dire échouer un examen.
On dit plutôt échouer à un examen :
- Il a échoué à son examen ce matin.
On pourrait aussi dire, sans préposition à :
- Il n’a pas réussi son examen.
- Il a raté son examen.
- Il a loupé son examen. (langue familière)
